SQL storage: drop the entity tables

The entity tables have not been touched for (literally) years.

The metadata work so far has been focusing on origins and visits as anchors for the data we'll retrieve.

I'm tempted to drop the entity stuff until we have a plan to fill it up with reasonable data.
